The front cover of each booklet is printed with an open letter from Professor Louis J. Opit of Monash University, indicating the involvement of the School of Social Medicine at Monash University in the survey and assuring the anonymity of respondents.
One of the three booklets is printed in red ink, one in green ink and one in black ink. The booklets printed using red and green ink are otherwise identical, have not been used, and were (according to the cover letter) intended for distribution to an unspecified number of "young people who left school last year". The booklet printed using black ink contains the same questions as the red and green booklets, but was (according to the cover letter) distributed to a sample of 5000 voters in Victoria "randomly selected from the elcetoral roll". This survey has been completed, and is inscribed '0504' on the front cover.
